An excellent question.  Your’s is a very common issue. The challenging issue for you is the tip of the nose. It would benefit for reshaping the cartilage and creating the structure that you reference in your question by grafting cartilage into the tip. After reviewing your photos, I believe a fat transfer in conjunction with a deep plane facelift would benefit you.  Though it’s not imperative, as you still have good volume, your best result with be with a subtle amount of fat added to select areas of your face.  With your anatomy, you should have a tremendous result.

At your phase of recovery it is way too early to be alarmed by swelling.  By all means you should discuss your concerns with your surgeon, as there are non-invasive ways of managing post-operative edema. For a rhinoplasty you should wait several weeks, if not several months, before drawing any conclusions as to your long term result.

It would be rare but yes, it is possible. With the events you describe it is possible you had a hematoma, a collection of blood under the skin, that did consolidate into scar tissue. I recommend you see your treating physician as there are techniques that can minimize this issue.

Based on your comments the major issue you have is volume loss that was not addressed during your surgery. Aging is a complex issue with three predominant factors: falling tissue, volume loss and surface aging. When we manage our aging face population we treat these issues with surgical face and neck lifts, fat transfer for volume loss, and laser resurfacing for skin aging. I believe the latter two would greatly benefit you.

Though difficult to fully assess through the photos you provided, lengthening your nose for a better overall contour is definitely attainable. Perfect nostril symmetry on the other hand is very challenging, if not impossible in most patients. A final recommendation would need to be done by a physical examination of your nose by a surgeon who specializes in rhinoplasties.
