When is it safe to go in a sauna/jacuzzi/steam room after facelift and why is it NOT wise to go
Skin redness (erythema)
Jan 2, 2020
Skin redness is caused by the capillary bed of vessels in the skin.  The degree of redness is related to dilation of the capillaries and can be a result of emotion (flushing), medication (vasodilator drugs), disease (rosacea), trauma (broken capillaries such as bruising), warmth (sauna), alcohol, etc to name a few.  Other causes include age and heredity.  Healing after surgery will cause erythema and this can be aggravated  by warmth.  In the early phases of healing after surgery erythe...
How common is tachycardia during rhinoplasty and can it be managed?
Tachycardia during rhinoplasty
Dec 16, 2019
Tachycardia during almost any surgery is common because of the anxiety factor.  In addition epinephrine, topical anesthetics (e.g. cocaine) etc can add to the rapid heartbeat.  This is manageable in most instances and rarely leads to other problems.  Death is extremely rare and can be related to underlying heart problems.  This should be discussed with your surgeon, but do not be fearful (it aggravates the problem).  The proper use of these drugs is very helpful and in some instances is es...
Would rhinoplasty significantly improve my appearance and be worthwhile?
Rhinoplasty considerations
Dec 16, 2019
Remember that cosmetic surgery is for the patient, not other family members, friends, etc.  If you feel that something should be done, then: is it safe, effective, affordable and will it satisfy you?  The answer in your case is likely YES.  There are other considerations that require a consultation with a qualified surgeon to address with you.  For instance limitations are present (skin thickness, draping over the revised bone/cartilaginous framework, etc.)  The most common reason for an un...
What can be done to fix a chin bulge and neck fibrosis after liposuction?
Neck liposuction swelling
Dec 12, 2019
The time frame is important in this case.  If within 3 months massage (?lymphatic massage) would be helpful.  Some authorities would recommend arnica montana orally.   Localized spots would probably respond to steroid injection.  Time is also your ally.
Will a mini work for me or would a full facelift be better?
Mini facelift
Nov 4, 2019
"What is in a name?"; Shakespeare asked.  There are so many variations in the "mini" facelift that it is much more important regarding what is done rather than what it is called.  You need defatting of the neck as well as tightening of the neck and face.  This is not a "mini" facelift by my thinking.  As an interim measure, thinking that a full facelift will be necessary in the future,  I think a "FaceTite" radio frequency procedure might be a better choice which would melt fat as well as m...

Vascular laser treatment compatible with Accutane?
Sep 7, 2019
I suspect that the treatment would be safe although the accepted response would be to wait for 6 months following cessation of the isotretoin.  A test spot would provide evidence of safety.  Ask your treating physician if a test spot could be done.
Looking for treatment of mouth wrinkles. Doctor told me a facelift would help. Thoughts?
Cheek creases resulting from smile
Sep 7, 2019
I think that any of the treatments that were mentioned will help.  None will totally defeat these lines which are the insertion of the bucallis muscle into the skin.  Fillers are probably the best "improvement" but not a "cure".  I would not recommend a facelift for this.
I had upper blepharoplasty last November 2018. But it is bumpy and and with scar. Should I have
Blepharoplasty result, Revision necessary?
Sep 5, 2019
An in office exam would be necessary to answer this question. While the photo would seem to show some excess skin that is no neccessarily the case. Inherited anatomy (shallow orbits for instance) may make the skin look excessive but further excision may lead to exposure problems. Brow ptosis (droop) also may make the upper lid look excessive but again not an indication for further excision. Discuss this with your surgeon.
Can PDO threads in the glabella or forehead cause blindness?
PDO thread lift of glabella causing blindness?
May 28, 2019
Although injection of filler into the glabella has been documented to cause blindness due to embolization of periorbital vessels this is extremely rare with proper precautions.  I am not aware that placement of threads in this area would cause blindness although swelling, bruising and pain can occur.

Nasal distortion following revision rhinoplasty 16 days ago
May 28, 2019
The photos are not clear enough to evaluate.  It is likely that this distortion will recede as the swellng abates.  You should visit your surgeon for reassurance.

Emsculpt treatment during pregnancy
May 28, 2019
Emsculpt treatment is contraindicated during pregnancy.  The magnetic energy of Emsculpt might have an effect on the embryo or fetus as it has a reported depth of 7cm. penetration.  Treatment following pregnancy might have a tightening effect on the abdominal wall so it might be beneficial to wait.

Thermi procedure causing numbness
May 28, 2019
The patient is now 2 months following a radiofrequency thermic procedure. He has numbness of the lower face. Nerve damage is very common following these procedures. Fortunately most of this will return although there could be some mild permanence. Massage generally will help this resolve faster. Other things including low-level laser therapy (photobiomodulation - PBM) might help although this is probably overkill. Massage is probably recommended. Painful areas also known as Tinel sign can b...

Face loosening following facelift
May 28, 2019
There is some natural loosening following a facelift as the swelling recedes and the skin stretches.  In fact there is a report in the lierature suggesting thaat some patients may meed a second tightening 6 months or so after a faceift as a result of this.  This likely would be very minor.  In fact you may want to explore the possibility of FaceTite RF tightening for the lower face/neck laxity.
How to reduce under eye puffiness or uneven zygomatic muscles? I have asymmetrical face muscles
Eyelid fullness, isometric
May 21, 2019
The patient complains of an asymmetrical fullness below variety. Photographs do not illustrate the problem well. Although the patient assesses the problem as being associated with zygomaticus muscle there are a number of possible explanations for fullness below the eyelid including herniated fat, malar fat pad shifting, atrophy of the underlying tissue, festoons or hypertrophy of the periorbital muscles, etc. This needs to be assessed by a competent plastic surgeon who can recommend possible...
Can cheeks which have been lifted too high during a facelift be lowered back down?
Cheek fullness following facelift - can it be revised?
May 21, 2019
The patient is concerned that a year following a facelift and her cheeks are still to full and she is wondering if they can be lowered. This concern is fairly common in a person who has excessively full cheeks and a wide face. As the tissues are restored to their youthful position with a facelift there is increased fullness along the malar and pre-auricular area. Almost always I advise against removing fat from face (not the neck) because there's a natural tendency for the face to deflate wi...
