If you are very thin and the implant is too low, this can be seen right away. Typically we sit patients up to evaluate the shape and look of the breast before leaving the OR.

Great question, these sutures dissolve. I’m not sure there’s a single surgeon who would use permanent sutures for this aspect of the surgery. It is rare that we use permanent sutures anymore for a TT.
Will I be fine without the Galaform internal bra?

Thank you for your question and photo. With the current shape of the breasts, a lift is necessary to provide a good outcome. A lollipop lift or anchor lift may be necessary depending on how much excess skin you have and what size implant is chosen. Using an “internal bra” mesh on a first time augmentation is typically not necessary. This adds extra costs that are typically not necessary. The internal bra is great for implant related issues if the original surgery was not done well.

Thank you for your question. Best way to think about a double bubble is that it’s a structural problem:  the implant has slipped down under the supporting ligaments of the breast.  This type of problem needs a structural fix:  repairing the bottom attachments of the breast.  Fat grafting does not repair this and is a mere “cover up” of the underlying problem.  For small double bubbles, this can sometimes be reasonable.  However, for most problems, it will fall short of a good fix.  Best wishes.

Thank you for your question and photos.  You do not have tuberous breasts.  However, as you have pointed out, an implant will help to create a rounder and fuller shape to the breasts.  An implant will generally make the breast larger, but choosing an appropriate size for your desires will help ensure a beautiful result.Best wishes.

Thank you for your question and pictures.  The color of the scar will continue to fade.  The width of the scar will not likely change much.  There are many dynamic forces on the breast and scar which ultimately lead to the appearance of the final scar.  The good news is scars can always be revised, we often encourage patients to wait between 6-12 months to ensure all changes have settled.  Best wishes.

Thank you for your question and photos.  You have a great deal of breast volume to start with.  A lift will use that existing volume and re-shape the breast to give it a more youthful look.  It’s often a great place to start if you are not interested in being bigger, which is what an implant will generally do.  You can always add an implant later if you want/need more fullness.Best of luck!

Thank you for your question and photos.  Overall the breast has a nice shape and fullness, but the lower pole is a little “tight” or mildly “hypoplastic”.  To really round this out, an implant will be best.  For often a slightly lesser fullness or roundness, fat grafting can be a great tool, which allows you to avoid the use of an implant. 
Can a hiatal hernia be repaired at the same time with a TT & BBL, by the same surgeon?

Thank you for your photos and question. A breast lift alone will likely not get you to your desired result as it will not be able to shape the breast as well as an implant.  The tuberous breast is “constricted” and the implant can help to push the breast out and round/fill it out more.  A lift can then help re-position the nipple and reduce the puffiness there.  Best wishes

Thank you for your questions and photos.  A muscle repair is quite standard with a tummy tuck as this tightens the rectus muscles that have been separated by stretch of the growing baby.  Some patients have a much more significant stretch of their muscles and others much less, but this is still generally done.  Liposuction is typically added on to improve shape and contour in some areas and is not “needed” but can often improve the final result.

Thank you for your question.  In general, most patients can express some “physiologic” fluid from the breast.  For some it’s minimal and for some it’s more than that.  The key points are a return of the breast to pre-pregnancy size and minimal to no nipple output.  This usually takes 2-3 months but at times can take up to 4-6 months for some individuals.  Bottom line, if it’s minimal, likely ok to proceed.

Thank you for your question and photos.  Lipo 360 w/ BBL by itself yields quite different results than the addition of a tummy tuck.  A tummy tuck is designed to tighten the muscle of the abdominal wall that have been stretched out from child bearing as well as manage the excess skin.  From your pictures, you don’t have a ton of excess skin, so making sure that your expectations are in alignment with the results are key to being a happy patient after surgery. Thank you for your question.  A cardiomyopathy is a general term and the degree of function is the most important factor.  Your cardiologist who follows your heart is the best one to discuss this with.  If you do move forward, having the procedure done in a more controlled environment like a hospital may be wise, depending on the degree of the cardiomyopathy.  Best wishes
