Yes your concerns are legitimate, and almost everything we do is a trade of shape and or size for scar. When we are just augmenting a breast without lifting it, we don't need to excise skin, so we just need enough of an incision to get an implant in. But whenever we are lifting the breast in a significant fashion, either for a lift or a reduction, some skin excision is required, hence the additional scarring. For small lifts you can often get away with less of a skin...
