Thank you for your question. You are an excellent candidate for a face and neck lift to treat the tissues that have descended with age. You would require deep contouring in the neck and jowls to achieve a more defined jawline-that would be achieved by fat removal, platysma muscle tightening and skin removal. The SMAS layer of the face as well as excess skin removal would improve the appearance of the cheeks area. 
What are my options regarding rhinoplasty? Would my nose be a good applicant for this surgery?

You are a superb candidate for rhinoplasty. Using 3D imaging during a consultation would help you see the planned shape and size of your nose after the procedure. A board certified plastic surgeon would help you determine the correct operative goals and design a plan to help you reach them.

Nice to meet you. An in person consultation is the best way to assess buccal fat (in a smiling photo it is difficult to determine the extent of the buccal fat). Your nose could be made softer/more feminine with subtle refinements-during my consultation I use 3D imaging so that we can make a plan that gives you the result you are looking for.

I would be very reluctant to have a non-surgical rhinoplasty at the tip of the nose as skin necrosis can occur. I do perform non-surgical rhinoplasty but the tip is considered a danger area. Tip refinements are best made with a traditional rhinoplasty as this tends to be a permanent change and is very safe to perform.

This is the most strict, difficult board for a surgeon to obtain certification. It requires a full plastic surgery residency (3 to 6 years of plastic surgery training), followed by a written exam, then another separate oral exam where your cases (mine included facelift) are reviewed by plastic surgeon examiners.

I just moderated a face and neck lift panel at a meeting with experts from around the world and the consensus is clear: there are no mystery or magical facelift procedures. Any procedure that involves deep tissue work and removal of excess skin, with fat grafting to restore volume, is a gold standard technique. The attention to detail and artistry of the surgeon as well as individualizing the procedure to each patient is what separates a good from a superb result.

An implant alone will give you volume and give an illusion of a slight lift, though an actual lift with an implant would likely give you better overall breast aesthetics. The “worm’s eye view” is the best for assessing the width of the alar bases. If your nostrils are significantly flared or wide than you may require one of the various designs for alar base excision. A short scar facelift refers to a variety of incision types that reduce the total length of incisions during a facial rejuvenation surgery. Classically this was described as having all of the incisions in front of the ear, occasionally going behind the crease of the ear. In general this procedure is an option for younger patients with minimal or no skin laxity in the neck. If there is a lot of loose skin in the neck, then a hidden hairline incision behind the ear will be required.

Healing after rhinoplasty does take many months. The amount of swelling between two sides can be variable. Nostril size is fortunately one aspect of rhinoplasty that if needing a revision at 12 months can be done without necessarily returning to the operating room. Alar retraction, however, is more problematic.
