I strongly feel silicone is manufactured as a superior product today. I have done 4500 pair, mainly saline..... And both me and my patients feel that the natural feel of the highly cohesive implant is superior. Also look for a surgeon who is using textured Sientra implants. The Data on decreasing capsular contracture rates is very impressive with this type of implant. Good Luck!!!

This does not looklike overuse!!! It looks like either blood in the pocket or a different fluid collection called a Seroma . A simple exam and sometimes an Ultrasound are needed. If this is blood or fluid and it is not removed.... A resultant hard breast could be expected( Capsular Contracture). Good luck and get back in to see your surgeon pronto.

This opening appears to be due to a lack of blood supply or tension.... Not infection. That dark tissue may need debridement. By cutting away the dead tissue you begin the healing process sooner. Keep a positive attitude and learn to do dressing changes twice daily. This will eventually heal with good conservative wound care. The ultimate scar won't be too bad or can be revised at a later date to make it look good . Good Luck. Dr.V

The answer to your question is complex. You should also consider if the implant is of a certain profile( low, moderate, or high). Your chest width matters. I have had excellent results filling soft tissue defects with a compound called polysilaxone (Silikon). It is highly purified and causes virtually no tissue reaction. To answer your question really has to do with what is bothering you most? Without a photo it is hard for us to tell. If you have skin laxity, stretch marks etc., you will likely need some skin excised. If skin is tight and you have some tough to get rid of fatty deposits, well then Lipo may be appropriate.

Based on the pictures you desire to resemble, and your current situation.....I suggest a Vertical Lift with Areolar reduction (Lollipop), I would use a high profile textured Sientra Implant over your muscle. My only concern is not the size of the opening, with good wound care that will close in a week or two. I am more concerned that a continuous running suture was used or a quill suture was used that a suture break may cause this long incision to split and open. For sure at minimum you should get back to your surgeons office and have them take a quick peek at it. Good Luck

Hi, thank you for providing photos. It allows each of the experts to personalize our advise to you. I feel you have PLENTY of volume and maybe you should consider a lift only!. It simplifies you situation. Adding a 475 cc implant to me makes your situation worse.At least consider avoiding the implant.

I would tend to lean toward waiting for a lift. You state you are DDD. Congratulations on the 80 lb. weight loss. You have had an amazing amount of change, remember scars on your breast are there for life! I would want you at a stable weight for 1-2 years, I would also have a serious discussion about removal of some breast volume because the weight of a DDD breast will likely stretch out the very skin that holds everything up. FYI creams, pills and exercises will never be able to...
