Breast base width is one of the factors used to determine maximal implant size. There are other factors involved such as amount of native breast tissue and the tightness of your skin. Putting an implant that is significantly wider in base width than the breast width can lead to an unnatural look and create multiple problems. Implant base width does not have to match the breast base width meaning you can choose a smaller implant size and still get a beautiful and natural result.

Based on your preoperative photos, I might have recommended a lift with augmentation. At this point, I would recommend that you wait 3 to 6 months to see how much your implants drop, then determine whether or not you need a lift. Based on your photos, I don't believe that a Benelli lift will give you the results that you desire. You will get more lifting with a circumvertical, short T-scar or other type of lift. Benelli lifts are good at diminishing areolar diameter, but don't lift very saggy breasts and the aureoles tend to stretch over time.

The amount of laxity that you have before surgery and the elasticity of your tissue will have more bearing on your final result than compression garments. Compression helps tissue with the ability to shrink do it more quickly, but it will not help if you have too much loose skin that lacks the capacity to contract.

Your surgeon is the best person to discuss size goals with. You have some breast tissue, so the implant will not need to be large to accomplish your stated goals. Show your surgeons pictures of women who have breast volume that is close to what you want to achieve and allow he/she to use intraoperative sizing to choose the implant size that meets your goal.

Based on your provided history and photos, I suspect that you developed a hematoma in the right breast. When this happens, most of the blood will eventually reabsorb, but if it is not drained early, the pocket can form on the inside of the layered old blood and end up being smaller than it was originally dissected. As long as your breast volume has not changed, your implants are probably okay. Sometimes if you have lost weight, the fluid in the implant can be heard when you are running since there is not as much tissue covering it as there used to be when your breasts were fuller. It would certainly be feasible to seek out consultation with a board certified plastic surgeon in your area for evaluation.

There are two points I want to make based on what I see in your photographs, a tummy tuck will remove some of the excess skin in the lower abdomen, but you look like you have a lot of intraabdominal fat that is causing a protrusion in the upper abdomen.
