Botox in the master muscles can slim the lower third of the face and also help with jaw pain from excessive clenching. Early asymmetry after treatment is common as the muscles adapt. A follow-up treatment 2 weeks later can help target areas where the muscle has overcompensated.

A crooked tip like yours often is due to both a crooked septum as well as crooked and asymmetrical tip cartilages (lower lateral cartilages). During a rhinoplasty procedure, both your septum and tip cartilages can be modified to improve the symmetry of your tip and nostrils.

A dorsal hump reduction can be performed at the time of a septoplasty, turbinate reduction, and nasal valve repair. This combination of procedures is very common in my practice. If your goal is to leave the tip of your nose alone, your surgery would not be considered "intense" in any way.

Surgery to correct a deviated septum is often covered by insurance. However, any additional cosmetic changes that a patient desires are not covered by insurance and are billed directly to the patient. In my practice, patients undergoing surgery to improve their breathing who also have cosmetic concerns usually pay between $6,000-$10,000 dollars. The price you were quoted does not seem unreasonable.

I would not expect there to be any damage to your facelift results 6 weeks after surgery. By 6 weeks the incisions and deep tissues should have healed well enough to withstand such an injury. I would still recommend you follow up with your surgeon so that he or she can assess your face and determine if any injuries are present.

Based on your concerns and goals, you appear to be a great candidate for a facelift.  Although I would also offer you fat grafting to improve your overall result, you would still see nice results if you only chose to proceed with the facelift. Obviously a consultation in person is essential before giving formal advice.

Incisions on the columella can have a certain degree of swelling over the first 1-2 weeks. I would allow your nose to heal further before becoming overly concerned with any asymmetries in the area. Your nostrils were quite asymmetric before surgery so some degree of asymmetry after surgery might be expected.

In your case, your profile could be reduced and your tip refined and deprojected to create a better balance with the rest of your facial features. I routinely perform rhinoplasty for a bulbous nasal tip. Many patient are bothered by a tip that is too round, ball-like, or boxy. In most cases, other changes need to be made to the nose (bridge) in order to make sure the new tip is in harmony with the rest of the nose. On occasion, I will perform a tip rhinoplasty without making changes to the remainder of the nose. A full physical exam would be necessary in order to be able to give you further recommendations for your nose.

Irritation of the eyes immediately after rhinoplasty could be due to a corneal abrasion suffered during surgery. Irritation that develops two months after rhinoplasty would be unlikely to be related to the rhinoplasty. However, a through history and physical exam is the only way to know for sure whether or not your symptoms are related to your recent nasal procedure.

When a dorsal hump is removed, particularly in patients with short nasal bones, a situation can be created where the middle portion of the nose "collapses" unless special cartilage grafts called spreader grafts are used to support the middle vault of the nose (internal nasal valves). Also, when the nasal tip cartilages are aggressively narrowed, the area of the nose above the nostrils (external nasal valves) can collapse inward unless care is taken to preserve support in this region...

It sounds like you are noticing an irregularity in the bone. When a bony hump is removed, rough edges or ridges can be created which can often be felt with the finger.  In some cases an "open roof" deformity can be created if bone on the bridge is removed and the adjacent nasal bones are not moved towards the midline to close this gap. Share your concerns with your surgeon who will have a better sense whether this is concerning or a normal finding.

Although most sutures used in facelift sutures are dissolvable and will dissolve over 3-6 months, some surgeons use permanent sutures. It is best to discuss your symptoms with your surgeon as he or she will know what sutures they used and whether the sutures are the likely cause of your pinching sensation.
