Early removal of a cast can certainly be the right thing to do, depending on what was performed. When a hump is removed and the nasal bones remained stable during the process, I also tend to remove the cast early, sometimes on day 3. Early removal of your cast may be an indicator that your surgeon was able to correct the shape of your nose with particularly soft techniques, so this may be more a reassuring rather than a concerning thing for you.

Dear Patient:
Alar strut grafts may be too firm and render the nal tip rigid. In combination with residual swelling of the tissues, your nasal tip may feel hard. Alar strut grafts can be revised and, if needed, replaced through an endonasal approach. Depending on the needed changes, this may even be done under local anaesthesia – together with fascial grafting.

Hi,
in principle, most changes are reversible. When it comes to bringing down an upturned nasal tip and / or other changes, it is important to balance the need for extensive grafting with the goal not to create a stiff nose. The endonasal approach may be preferable in these cases, at least in my hands. Imaging is certainly helpful to understand your goals and to see if they can be met. Best regards, Holger G. Hi,
the choice of the right transplant material for camouflage is important. Often, the surgeon decides during the case which material is best suited. Options inlcude thin fascia from the temporalis muscle or from other sources, perichondrium, diced cartilage and many more. I would advise not to get too involved in your surgeons´s decision. Your nose looks contracted and sceletonized on the photos. It would be important to very diligently examine your nose, especially the skin. In principle, it should be possible to revise your nose, make it look more harmonious and less “operated”. Grafting e.g. of ear cartilage may be required to obtain the result you desire.

Your nasal tip does look large, round and turned downward on the photo. Some of this may still be edema. If you have phases with less swelling, this would be an indicator that your tip may still become smaller. A thorough evaluation can be done after about 10 months. Revision of your tip may include turning it slightly up, making it appear less round and less wide. This can be done through an endonasal approach. For now I would advise to be patient and re-assess in 3 – 4 months.
