This patient had an over-projected nose with a hump. Projection refers to how far forward the tip of the nose projects off the face. In this case her over-projection resulted in a tension tip. A tension tip is when the nose projection pulls on the skin to an extent that it draws the upper lip forward with it. In the after photo you can see how the upper lip has settled backwards into a non-tensioned position as a result of reducing the projection of the nose. When the nose projection is reduced, the hump becomes even more pronounced which requires removing more dorsum than otherwise. Similarly, if your were to increase projection, you would need to keep more dorsum (or add dorsum which is often the case in Asian rhinoplasty). (Jan 22, 2020)
