Am I a good candidate for tear trough fillers?
Filling Lower Eyelid Tear-Troughs
Mar 2, 2020
You have what I would call moderate tear-troughs.  Injections of Restylane work very well for mild tear-troughs and in fact is generally all that I suggest.  Moderate tear-troughs typically experience a nice improvement that is well worth the effort, but rarely is considered a complete correction.  Severe tear-troughs typically just don't get a worth-while result from injections.  Moderate and severe tear-troughs both do well with surgery.  Between 1/2 and 1 cc is appropriate for each eye....

Hello,You appear to have levator ptosis on the left upper eyelid.  The root cause of this is a poor connection between the eyelid opening muscle and the eyelid structure.  With this stretched connection, the muscle "tries harder" to open the eye pulling back deeper into your eye socket, drawing fat and skin deep into the eye socket along with it.  That causes the long, hollow eyelid seen on that side.  I can not see your eyebrow on the left, but commonly that eyebrow will try to "help ou...
What is better for flat cheekbones: Radiesse or Sculptra? Can they be combined?
Cheekbone Filling Radiesse, Sculptra, or Hyaluronic Acid
Jan 22, 2020
When it comes to facial volume injections, it is my opinion that the best option is a hyaluronic acid product.  These include many brands and "thicknesses", I use Voluma and Lyft for cheeks.  I am not in favor of using Sculptra.  Serious complications are vastly more common and very difficult to treat.  A well known complication is granuloma formation which may need to be surgically removed and can last decades.  Scultpra relies on your body reacting to the product to produce any result. ...
Do fillers migrate but never fully go away?
Do fillers never go away and migrate?
Dec 16, 2019
This all depends on what filler is used.  The vast majority of fillers used are made of something called hyaluronic acid (HA).  Juvederm and Restylane are the two most common brands of HA.  These products will dissolve in time and will not lead to jowling or sagging.  HA doesn't migrate, you can "push out" the edges with massage early on, but it won't altogether move to a different location.  HA fillers can be dissolved on purpose when using a product to dissolved it (hyluronidase).  The dis...

Soft Bulbous Nose Rhinoplasty
Dec 16, 2019
Hello,An exam is needed to really define what needs to be done, but based on your photos and my experience, I am expecting that your nose is very soft and has thick skin.  To provide a narrower, more refined nose, I'll need to strengthen your nose.  Narrowing techniques by themselves won't work when applied to a soft framework.  The thick skin can't really be changed, but a good underlying structure can largely compensate for the loss of definition that thick skin imbues upon your nose.  ...

Asian Lower Eyelid
Oct 29, 2019
Hello,The photos are tight to your eyes, so I can't be certain of your facial expression.  Focusing on the second two pictures where you appear to not be smiling I see a flat, hollow, long lower eyelid.  The front of the cheek bone is commonly flat like this in people of Asian genetics.  Regardless, the long hollow lower eyelid often makes one look tired and/or sad.  Augmenting that under eye portion of the cheek bone with a tear-trough implant will produce a fuller more convex shape to th...
Is it possible to perform an otoplasty on someone whose bones protrude above the ear?
Protruding Ears and Otoplasty
Oct 29, 2019
Hello,It is hard to know without photos and an exam.  If your ears are currently tight up against the bone, then probably not much can be done.  I can not rule out the possibility that bone could be removed to allow for more space to bring the ear back, but probably not.  The bone behind the ear is referred to as the mastoid bone.  This bone contains a honeycomb like network of air pockets in it.  If you grind down the bone you would enter into these air pockets opening up all sorts of issue...
Are my cheeks asymmetrical? If yes, are fillers an option?
Facial Asymmetry - One Side of Face Smaller Than Other
Oct 29, 2019
Hello,Everyone has some facial asymmetry, it is only how much asymmetry that separates one person from the next.  The asymmetry can be primarily the result of differences in the bone, soft tissue, or both.  Often times I see people where one side of the face is overall smaller than the other.  In these people the smaller side of the face will demonstrate a lower eye position, a deviated chin to that side and a nose that curves toward the smaller side.  This is the most common cause of crook...
What incision is the best decision for my lower eyelid?
Lower Eyelid Incision
Sep 24, 2019
Hello,To access the fat of the lower eyelid it is best to make an incision behind the eyelid.  The vast majority of people should not have skin removed from the lower eyelid.  In the rare patient that needs skin removed, then a skin incision is needed.  It is important to understand that removing wrinkles from the lower eyelid is not done with skin removal.  Skin removal is only needed for people with folds/flaps of skin at rest.  Wrinkles in the lower eyelid skin are treated with resu...
Unsure what makes my face asymmetrical and what procedure would help?
Facial Asymmetry - One Side of Face Smaller Than Other
Sep 19, 2019
Hello,Your situation is VERY common.  No one has perfect symmetry, the question is not if you are asymmetric, but rather how asymmetric are you.  There are lots of ways to be asymmetric, but I would say the most common asymmetry for which patients request evaluation is where one side of the face is smaller than the other.  In your case, the right side of your face is smaller than the left.  The smaller side of the face will have a lower eye and eyebrow position, a higher angle of jaw, the n...
Can I get Botox/Dysport during my period/menstrual cycle?
Botox/Dysport/Jeuveu or any botulinum product relationship to your menstrual cycle
Sep 17, 2019
Hello,There is no interaction between any of the botulinum products and their use with regard to the female monthly cycle.  There is also no interaction with regards to any of the methods to modify that cycle such as oral contraceptives, IUD, hormonal implants, hormonal patches, hormonal injections, hormonal ring, or other similar methods. You may choose to time your treatment in your cycle if you find it to be more tolerable at certain points, but that choice will not affect the resu...
What are these dents under my eyes, on my inner cheeks? How can I fix them when filler hasn't?
Tear-Troughs
Sep 17, 2019
Hello,You have tear-troughs.  Tear-troughs are primarily a bone shape issue and secondarily a soft-tissue problem.  Tear-Trough implants are an excellent way to effectively produce a change to the shape of the bone.  They are permanent (but removable).  They look and feel like normal, you can not move them or tell they are there (except when comparing to pre-op photos).  Soft tissue fillers can help, but not nearly so much as an implant.  If you are going to continue using injection...
Is deep temporal fascia a good augmenting material for increasing tip projection and lengtheni
Increasing Nasal Projection
Aug 20, 2019
Hello,Increasing nasal projection is challenging and requires the use of strong materials.  People who need projection usually have naturally soft "squishy" tip cartilage.  Stacking more soft material (like DTF) over the tip of this weak structure just squeezes down those weak underlying tissues instead of adding projection or length.  I use structural cartilage grafts that reinforce the natural tip cartilages allowing for additional cartilage grafts to then be placed over the top of the n...
Will a half of syringe of filler for cheeks make a difference?
Volume Restoration in Cheeks
Aug 20, 2019
Hello,Generally speaking, 1/2 cc of any filler in the cheeks is going to produce very little change, though there are exceptions.  Basically, if you are putting the filler in a bony area that doesn't have a lot of overlying soft tissue, 1/2 cc will produce a result.  Imagine putting a marble on a hard table and then laying a silk sheet over it, that marble will be obvious.  Now imagine putting that marble inside a pillow, you won't see it at all.  The areas where small quantities are mos...

Lower Eyelid Filler vs Surgery
Jul 29, 2019
Hello,Based on your before pictures, I would not have recommended HA filler (Restylane).  Simply put, you have very deep tear-troughs.  Restylane (or similar) is very good at treating mild tear-troughs.  Though it is an area that only a very skilled injector will consistently get good results.  Between 1/2 and 1 cc into each tear-trough should be the maximum used.  If you need more, then you need something else.  The results on mild tear-troughs are usually excellent and usually last at le...
