Hi, I understand it is difficult to go through so many surgeries and still feel unsatisfied. I can suggest a few options but I am making judgments based on photographs which are not very reliable when compared to an in-person consultation. One option is liposuction but it's hard to say if that will adequate treat the right side. You may need fat grafting for the left side to improve the appearance of the nipple. You may get a better result from skin excision on the right side....

The short answer is yes but it is worth considering the pros and cons. HRT can increase nipple and breast size and potentially accommodate a larger implant. Of course HRT has its own side effects that some patients are not fond of. When undergoing breast augmentation without HRT, I recommend smaller implants since the pocket is tighter and the nipple size is smaller. Overall breast augmentation is a procedure with a high satisfaction rate.

In facial feminization, I believe it is important to analyze each part of the face to determine a treatment plan. Most patients benefit from cheek augmentation, chin augmentation, rhinoplasty, forehead reduction, and lip augmentation. Botox in my opinion is meant to treat wrinkles and does not really feminize the face. There are a series of procedures under the category of Facial Feminization that can help feminize the face. Some patients desire doing one at a time but other have multiple procedures done at once. The key is the procedures should be done safely in an efficient manner by a board certified plastic surgeon.
How to Reduce Pain and Recovery Time After Breast Implants Surgery?

I minimize pain using a number of measures. One is using local anesthesia and providing pain medication post-operatively. I prefer that women take very few if any narcotics after surgery. The other key to reducing pain is having a technique that is very clean and minimizes blood loss. Hope this helps.

Great question. There is no formula or hard and fast rule for picking an implant size. I always tell my patients to look at pictures online and try to find sizes that they desire. Then in the consultation, they bring in these pictures so we can have a frank discussion about sizing. We do use sizers and I make measurements to help guide the decision making process. Ultimately I try to agree on a size with the patient that will make them happy and meet their goals.
