Breast droop is called ptosis , ptosis can range from severe to moderate. You seem to have a moderate level of ptosis. Position of your nipple is good. If you would prefer your breasts to sit higher on your chest , you may consider Polyurethane coated breast implants (PBI) placed in the sub glandular position. Polyurethane breast implants biointergrate with your bodies own tissues effectively acting like Velcro preventing breast ptosis in the long term.

Your surgeon is perhaps correct in saying a “standard” breast augmentation would not provide a suitable solution to your tuberous breast condition.
Tuberous breast have a very narrow breast base and your right breast seems to have a higher inframammary crease. It would seem that you would require a prosthesis that will broaden your breast base and the right breast inframammary crease would need to be lowered.
