Best strategy for mini lift, upper maxilla/mid face augmentation, rhinoplasty and eyelid lift?
Multiple facial cosmetic surgery procedures is possible in the right hands and the right patient
May 24, 2020
Healthy patients can safely undergo multiple facial procedures as an outpatient and the list you present is not unreasonable as a single stage. In the right surgeon’s hands it’s not necessary to team up with other surgeons unless one or more procedures is outside the surgeon’s skillset. Finding the best surgeon is the key! Aside from board certification in plastic surgery, training at the best institutions, and at least a decade of experience after training, patient reviews and surgeon’s b…
How Do I Minimize Bruising from a Rhinoplasty?
Closed rhinoplasty is minimally invasive
Oct 27, 2018
Hi,Bruising is multifactorial: 1. Your tendency towards bruising – this is hereditary. How to avoid? You can’t change your genetics.2. Medications such as NSAIDS (Ibuprofen, Advil, Aleve, Motrin, etc.) and blood thinners (coumadin, Lovenox, etc.) that predispose to bleeding/bruising.How to avoid? Don’t take these medications within 2 weeks of surgery.3. Surgical invasiveness: open rhinoplasty is more invasive as compared with closed rhinoplasty. How to avoid? Closed rhinoplasty minimizes…
How can I find the best rhinoplasty surgeon?
How to find the best surgeon
Oct 27, 2018
My advice in how to find the best rhinoplasty surgeon is the same for any procedure: Credentials are critical: training at top institutions and Board certification in Plastic SurgeryExperience is everything: preferably a decade or more of high volume aesthetic surgeryCommunication: your surgeon should spend time with you to ensure you are well informed and expectations for the result, recovery and risks are aligned. Results: reviewing multiple before/after results gives you a sense of the su…
Would I be a candidate for bulbous nasal tip correction?
“Closed” rhinoplasty is effective, scarless and 7 days of recovery
Oct 27, 2018
Hi, Yes!  Your nasal tip can be narrowed and refined by performing “cephalic trim” (removing the wide part of the tip cartilages), and tip cartilage narrowing.  You can enjoy a beautiful result with a “closed” (aka “endonasal”) rhinoplasty approach which involves hidden incisions within the nostrils, no visible scar, minimal swelling/pain and less than a week of recovery. Look at my photos for examples of wide, bulbous tips. Open rhinoplasty approaches are suboptimal as they involve a visi…
I am considering a face lift and want feedback on my eyelids. I feel like they are getting heavy.
Upper and Lower Blepharoplasty should accompany Brow Lift and Facial Fat Grafting
Jun 9, 2018
Hello- While your photo is shadowy and difficult fully to assess your anatomy, I can tell you have naturally beautiful facial architecture: full cheeks, heart shaped face, and excellent symmetry. Focusing on your upper and lower eyelids, I see that you have some excess upper eyelid skin and low positioned eyebrows which can be improved with combination brow lift and upper blepharoplasty. It’s important to consider both procedures together because the brow lift reduces the amount of upper…
66 looking to get a facelift. should I look for a Board certified plastic surgeon or a facial boa
Training is essential, but talent is hard to measure
Feb 10, 2018
Hello,Certainly the quality of training and Board certification are essential, but artistic and technical ability transcend these basic criteria and are diffi. Decide whether a surgeon’s long term before/after photos speak to your own aesthetic for the results you seek. Speaking with the surgeon’s patients can give valuable insight about his/her attentiveness and a realistic idea of recovery. Finally, a referral from friends or acquaintances who’ve are happy with their results, or who are f…

Hello,A “donut” (called Benelli) lift won’t give you the result you want. Instead you will benefit from a “lollipop” (called vertical) breast lift. The lollipop technique is very reliable in experienced hands and the scars fade well. Although eliminating the vertical scar is possible in certain patients with very low, droopy breasts, your degree of breast droop is not amenable to this technique particularly with augmentation, as well. In performing augmentation/lift on my patients, the key i…
Which would you recommend; Under or over muscle implant?
Cost should be the same, and under-muscle is safer
Feb 10, 2018
Hello,In my practice cost does not change with above of below muscle implant placement. It has everything to do with safety.  In looking at your anatomy, a breast lift is definitely needed with your augmentation. For safety reasons implants should be placed under the muscle in order to preserve the blood flow to your nipples and breast skin to ensure proper wound healing. Under-muscle placement also protects the nerves to the nipples so that numbness is less likely. Best of luck!
Over or under the muscle?
Pros and cons to implant placement are many
Feb 10, 2018
There are pros and cons to above-muscle and under-muscle placement of implants in primary (first time) breast augmentation. The choice depends on unique anatomy. Above muscle placement is best if the breast tissue and nipple are mildly to moderately droopy and the patient does not want a breast lift. Recovery is easier, but risk of capsular contracture is higher. Below muscle has a more challenging recovery but risk of contracture is lower. Aesthetically, more fullness in the upper bre…
Rhinoplasty Swelling
Patience is a virtue
Feb 10, 2018
Hello,The 3 main factors with persistent swelling are (1) if you had an extensive procedure (with osteotomies and major reshaping), (2) if you had an “open” rhinoplasty (visible scar underneath the nose), and (3) if it you had a corrective rhinoplasty, then swelling can last many months. Although certain easy things can help, such as elevation, icing and a taping regimen, the most important is time.  Patiently waiting for your tissues to heal is the only effective strategy.  Best of luck.
Post Rhinoplasty: Why Does my Nose Look Bigger Than Before Surgery?
Proportion is everything
Feb 10, 2018
Hello,In rhinoplasty proportion is everything. When the upper bridge (called “radix”) is reduced, then the nose can look longer and the tip larger.  Although swelling at one month after open rhinoplasty is common, it looks like the proportions of your nose have changed which is why your tip looks larger. If you still have concern at 6 months then a revision may be needed.  Best of luck.
Rhinoplasty Recovery Time and Downtime?
“Closed” vs “open” rhinoplasty: a big difference in recovery time
Feb 10, 2018
Rhinoplasty recovery depends on how invasive surgery is and I agree that it depends on the extent – for example, osteotomies (bone fracturing) causes bruising and swelling. However, when comparing techniques there’s a big difference in recovery between “closed” approach (hidden incisions, minimal dissection and the least down time) and the “open” approach (visible scar, maximal dissection and the most down time). Months of tip swelling and a visible scar, hallmarks of the open approac…
How Much Does Rhinoplasty Cost?
Rhinoplasty cost depends on many factors
Feb 10, 2018
Rhinoplasty cost depends on many factors including region of the country. In DC the cost of rhinoplasty ranges from $4500 – $10,450 with an average cost of $7600, and higher for corrective work.  However, price is less important than the training (did your surgeon train and top flight institutions?), Board certification (American Board of Plastic Surgery or Facial Plastic Surgery),  and experience (number of rhinoplasties performed annually). Finally, the technique your surgeon uses is i…
I like the front view of my nose but not the profile because I have hump. How much would it cost
Surgery is best for your needs to reduce the hump on your nose
Feb 10, 2018
Hello,Fillers are good for hiding a bump when the upper bridge is scooped out. Your upper bridge is not scooped and so fillers will have a limited effect on your profile. To get the best result you need a reduction of your hump and better definition to your tip. Only surgery can achieve this. Although fillers are cheaper in the short term, surgery is more cost-effective in the long term because you’ll have a lifelong result you can enjoy. Good luck!
Whether to get nose fillers? Or actual rhinoplasty? Wanting to slim my nose down.
Surgical rhinoplasty is needed to reshape your nose
Feb 10, 2018
Hello,Non-surgical rhinoplasty involves filler injections which are useful to hide a hump (which you don’t have), but there are not useful for slimming or or reducing the tip. A surgical rhinoplasty is best to address your concerns. I recommend a combination of tip cartilage reduction and tip grafts which will give the appearance of a slimmer, more defined tip. Adding grafts (cartilage) to the upper bridge (called “radix”) to make your tip look smaller. This can be done by filler, but sin…
