Based on your photos, age, and concern, I would recommend a short scar facelift with the MACS technique. This utilizes a more vertical lift of the upper neck and cheeks than a more traditional lift. There is no scar behind the ears as you don't seem to need any skin removal there. This is excellent for tightening, lifting, and smoothing the upper and lower jowl bulges, jawline, and temples. It is my preferred technique for younger patients who don't require any significant lower neck work.

Since I don't perform the "Necktight" procedure, I can't speak to that directly. However, based on your photos, I would suggest a standard necklift to lift and support your platysma muscles both under the chin and perhaps also laterally towards the back of your neck, as well a general redraping and smoothing of your neck skin. Unfortunately, you have minimal subcutaneous fat to cover the neck muscles, so there will likely be persistent visibility of muscle fibers to some degree.

Despite your young age, your photos do show more fullness in the anterior cheek, upper and lower jowl areas, and deepened nasolabial and marionette folds. I think a cheeklift or upper facelift, avoiding the neck, should be able to reposition fat and skin upward toward the lateral cheek prominence to create a more narrow lower face, a more prominent upper cheek, and thereby a more youthful facial shape. This would be a more vertical lift than sometimes used.

Generally, with a standard necklift, only the lower part of the jowl bulges, along the jawline and just above or below the jaw will be improved. Any jowl bulging up near the corner of your mouth or above won't be changed. Of course, the procedure can be customized to involve working a little higher in the mower or mid cheek, which would would afford improvement of jowls near the corner of the mouth or above.

My preferred type of anesthesia for facelifts, necklifts, and eyelid surgery is iv sedation often referred to as twilight anesthesia. This is supplemented with local anesthesia that is injected when the patient is sedated enough that they don't feel the injections. The patient rests comfortably and breathes on their own throughout the procedure. A thorough discussion with your surgeon and the anesthesiologist should consider if this is right for you.
